The Surveyor Certification Form, Local Government Verification of Contribution Forms, and Local Government Verification of Qualification as a TOD Development Form included within an applicable Request for Applications (RFA) are provided here in Interactive Portable Document Format (PDF) .

These forms include blanks or fields that can be completed by the Applicant before printing.

• These forms can be completed using Adobe Acrobat X or by using Adobe Acrobat Reader. Use of other software may result in errors. If Adobe Acrobat Reader is used, Applicants are strongly urged to use the latest version of Adobe Acrobat Reader, available free of charge at http://www.adobe.com.
• In order to save the completed forms, the Applicant must use Adobe Acrobat Professional. One cannot save documents in Adobe Acrobat Reader.

Applicants are not required to complete the interactive PDF forms before printing. These forms may be printed and then completed manually. Applicants are strongly urged to use the latest version of Adobe Acrobat Reader.

Applicants participating in an applicable RFA process are solely responsible for completing these forms in accordance with the applicable RFA instructions and ensuring that the information provided on these verification and certification forms is correct, complete and consistent, regardless of whether the Applicant chooses to complete the interactive PDF forms prior to printing or print the forms and manually complete them. As provided in the applicable RFA instructions, Applicants may be penalized for errors, omissions and inconsistencies created by their use of these forms.

The applicable form(s) must be included as an attachment in the Applicant’s submission in response to an applicable Request for Applications (RFA). Submission of the form(s) separate from the Applicant’s submission in response to an applicable RFA will not be considered.
